Students are encouraged to participate in the numerous extra and co-curricular activities provided at Merrill High School.  Students can sign up for a sport using Skyward Student Access a month prior to the 1st practice. Fall Sport Sign-up is open for registering. Here is the "How To" form that explains the process.

Prior to participation in extracurricular activities, the parent must fill out all athletic forms online in Skyward Family Access.  Here is the "How To" form that explains the process. This includes...

  • MAPS Athletic Code of Conduct

  • MAPS Parent Code of Conduct

  • MAPS Emergency Card

  • WIAAA Concussion Form

  • WIAA Eligibility Form

  • WIAA Alternate Year Card

Students that are in need of a physical, must turn in the WIAA Physical Form signed by a physician prior to the start of the 1st practice or they will not be allowed to practice.

Parochial and Home Based students will need to open the following form and print all forms and return to the Athletic's Office.

When transportation (bus, van) to school activities is provided, students must use that form of transportation unless a parent provides transportation with prior written approval by the principal, associate principal, or activities director.  Please print and fill out the Travel Release Form and give to your son/daughter coach.
